Unity Care NW ‘s enrollment services are free and unbiased. Our Enrollment Specialists have been trained and certified as In-Person Assistors by Washington State’s Health Benefit Exchange to help you apply for or renew your health insurance coverage or assist you in applying for our Sliding Fee Discount Program.

Apple Health Redeterminations

Apple Health Renewals were paused during the national COVID-19 public health emergency to help make sure as many people would maintain health care coverage as possible through the worst part of the pandemic. Since the national emergency declaration came to an end on May 11th, 2023 anyone covered by Apple Health will need to again file an annual renewal with their insurance provider. If you need help or have questions about filing your renewal, give our Outreach & Enrollment team a call. Our Enrollment Specialists are certified to help you with forms and to navigate the state’s health insurance exchange for free.

As your health care provider, we urge you to act to make sure you won’t miss a beat with your insurance coverage. The Health Care Authority website provides renewal information and instructions.

Sliding Fee Discount Program

To ensure that you have access to affordable care regardless of ability to pay, we also offer a Sliding Fee Discount Program that discounts charges for those who are eligible based on household size and income.

Call us to apply for the Sliding Fee Discount Program.  You will be asked to complete a Sliding Fee Discount Program Application and provide current proof of household income. If you have no income, you may fill out a Self Attestation of No Income form.
