Primary Care Team Approach

As your medical home, Unity Care NW is committed to a patient-centered care team model that engages patients and their families in their care. Our care team includes not only primary care providers and medical assistants but health coaches, mental health specialists and case managers as well.

Your Primary Care Provider

Each patient has a Primary Care Provider, or PCP. We try to schedule all of your medical appointments with your PCP.

Your PCP:

may be a doctor, nurse practitioner, or physician assistant.
will help you with referrals to specialists, Unity Care NW mental health professionals, or other providers, if needed.

Your Annual Wellness Visit

Your Annual Wellness Visit is a dedicated check-in focused entirely on staying healthy — not on treating a problem. It’s a benefit included in most health plans at no cost to you, and it’s one of the best ways to stay ahead of health issues before they start.

At your visit, your care team will review your health history, go over your medications, check key vitals, talk about healthy habits, and make sure you’re up to date on recommended screenings.

Wellness Visit vs. Problem-Focused Visit

These are two different types of appointments and both matter.

Annual Wellness Visit	Problem-Focused Visit
Focused on prevention and planning	For new symptoms, pain, or health concerns
Reviews your health history and screenings	Addresses questions about medications or treatment
Covered at no cost by most health plans	Scheduled as a separate visit
