To request transfer of your medical record to another facility or to request a personal copy of your medical record, complete a Release of Information Form.

Find out how your medical information may be used and disclosed. Unity Care NW respects your privacy, and we understand that your personal health information is very sensitive. We will not disclose your information without your approval or unless the law authorizes or requires us to do so.

Insurance Policies and Sliding Fee Discount Program

Unity Care NW accepts most insurance plans, including Apple Health (Medicaid), Medicare, and private insurance. To ensure that you have access to affordable care regardless of ability to pay, we also offer a Sliding Fee Discount Program that discounts charges for those who are eligible based on household size and income.

To apply for the Sliding Fee Discount Program, contact our Outreach and Enrollment staff at (360) 788-2669 or You will be asked to complete a Sliding Fee Discount Program Application and provide current proof of household income.  If you have no income, you may fill out a Self Attestation of No Income form.

About Enrollment Services

Unity Care NW is dedicated to helping our community access insurance coverage and learn about other options for accessing affordable care. Our Enrollment Specialists have been trained and certified as In-Person Assistors by Washington State’s Health Benefit Exchange and are available to help you through every step of the enrollment process. Our enrollment services are free, and the information you receive from our specialists about health plans is unbiased. For Unity Care NW patients who do not qualify for insurance coverage or who have high deductibles or co-pays, our Enrollment Specialists can assist you in applying for our Sliding Fee Discount Program and provide information about other available programs.

Open Enrollment

Open Enrollment is your opportunity to renew insurance coverage, change plans, or sign up for coverage for the first time. It is the only time of the year to sign up for qualified health plans or renew existing health coverage.

If your family is very low-income or your family has recently experienced a life event, such as having a baby or getting married, you may also be eligible to apply for coverage outside of the Open Enrollment period. Enrollment in Apple Health (Medicaid) remains open throughout the year, based on eligibility. Contact our Outreach and Enrollment staff for more information.

To schedule an appointment, please contact us at (360) 788-2669 and speak to one of our Enrollment Specialists. Although staff is occasionally available on a walk-in basis, we strongly recommend making an appointment.

Quality Improvement (QI)

Providing high-quality health care to our patients and continuously working to improve the quality of care we deliver are top priorities at Unity Care NW. We have adopted the Institute for Healthcare Improvement’s (IHI) Triple Aim framework to improve health system performance. We work to improve:

  1. Patient satisfaction
  2. The overall health of the community
  3. Cost effectiveness



Performance measures and organizational initiatives are updated annually and are passed by our Board of Directors. Additionally, we annually review and evaluate our prior year’s QI activities. Progress towards achieving our measures and goals are collected and reported quarterly to our staff, leadership teams, and Board of Directors.
