Primary Care Team Approach
As your medical home, Unity Care NW is committed to a patient-centered care team model that engages patients and their families in their care. Our care team includes not only primary care providers and medical assistants but health coaches, mental health specialists and case managers as well.
Your Primary Care Provider
Each patient has a Primary Care Provider, or PCP. We try to schedule all of your medical appointments with your PCP.
Your PCP:
- may be a doctor, nurse practitioner, or physician assistant.
- will help you with referrals to specialists, Unity Care NW mental health professionals, or other providers, if needed.

Sliding Fee Discount Program
Unity Care NW offers a Sliding Fee Discount Program to help keep care affordable for uninsured patients and those with high deductibles and co-pays. Eligibility for the Sliding Fee Discount Program is based on household size and income.
