LPN (Ferndale)

This LPN Lead serves as a Care Team Coordinator (CTC). The full-time benefited position assists in daily patient management in the UCNW Ferndale Clinic. The LPN Lead/CTC gathers patient information for RN triage, assists with walk-in patients, responds to telephone inquiries and medication refills, and manages EMR desktop items. On an ongoing basis, the LPN/CTC maintains the controlled substances spreadsheet, manages Behavioral Health controlled substance contract requirements, and manages the Arcadia Risk Conditions platform by coordinating appropriate activities to ensure open risks have been addressed by the provider. With the Nursing Department Supervisor, the LPN/CTC ensures in-house medications are ordered and inventory is managed, and orders lab supplies from QUEST and NW Pathology as directed. The position performs clinic support tasks, including sterile instrument management and backing up Medical Assistants as necessary. The LPN/CTC is part of a multidisciplinary patient care team providing and coordinating medical care in an outpatient setting following a patient-centered, primary care model.
