Health care happens inside and outside the exam room, Unity Care NW’s innovative Population Health department addresses the variety of factors that affect the overall health of the people and communities we serve.
We recognize that health outcomes are not just the product of individual behaviors or predispositions. Our Population Health team’s focus on advancing health equity is an invaluable component of our whole-person care model.
Patient Engagement Support
The Patient Engagement team addresses health disparities by providing uniquely tailored supports to patients who are falling through the cracks and advocating for improvements to the UCNW healthcare system. The team includes:
- Community Health Workers connecting with vulnerable patients
- A Hospital Community Connector stationed at Peace Health
- Case Management services to help patients access other community resources, such as transportation, food, and legal assistance
Outreach & Enrollment
Outreach and Enrollment Specialists are experts in Medicare and Medicaid and can help Unity Care NW patients apply for sliding fee discounts that enable them to live healthier and happier lives.
Health Promotion
Using evidence-based strategies, the Health Promotion team empowers patients to prevent and treat chronic diseases by applying individualized information mindful of tastes, culture and budget.
